The following documents are required for full-time enrollment in BLVS. Use the checklist to gather the documents you will need. Please have documents available and ready to upload before starting the online enrollment application in step 3.
❏ Proof of Age
Provide a copy of the student’s official birth certificate.
❏ Immunization Records
Provide Kansas Certificate of Immunization.
❏ Academic Records
Incoming Kindergarten Students

- No academic requirements apply.
Incoming 1st - 3rd Grade Students

- Submit all report cards received to date for the current school year.
- Submit available district assessments in English Language Arts and Mathematics.
Incoming 4th - 8th Grade Students

- Submit all report cards received to date for the current school year.
- Submit available Kansas Assessment Program scores in English Language Arts (ELA) and Mathematics OR submit available district assessments in ELA and Mathematics.
Incoming 9th Grade

- Submit all report cards received to date for the current school year.
- Submit available Kansas Assessment Program scores in English Language Arts (ELA) and Mathematics OR submit available district assessments in ELA and Mathematics.
Incoming 10th, 11th, and 12th Grades

- Submit an unofficial transcript, including all high school credits earned to date for the current school year.
- Submit available Kansas Assessment Program scores in English Language Arts (ELA) and Mathematics OR submit available district assessments in ELA and Mathematics.
Prior Year Homeschool Students

- For Kansas residents who lived out-of-state the prior school year or for homeschooled students, please contact the BLVS office for the requirements necessary to document your student's academic skill level.
** Official enrollment is contingent upon receipt of final academic records for the current school year.
